Terms and conditions

I understand that Breakthrough therapy and resources, Jeffrey Tucker LCSW, LCADC are out-of-network providers and that I am financially responsible for all  appointments to be paid in full at the time of the session regardless of reimbursement from my insurance company.  There will be a returned check fee of $35. Payment may also be made by credit/debit card or FSA debit card.  The cost of the initial assessment is $200 (approximately 75 min).  Weekly individual sessions are $175-250 (sliding scales) and family sessions, should they be required, are $350.  Treatment receipts will be provided that may be submitted for out-of-network reimbursement depending on your benefits.

 I must sign a release of information for my information to be shared verbally, by fax, mail or email with anyone other than employees or agents of breakthrough therapy and resources, Jeffrey Tucker LCSW, LCADC.  A release form is available upon request.  Your privacy is our number one priority. All sessions are considered completely confidential unless you give written permission.  If you are at risk of hurting yourself or others, or there is a suspicion of child or elder abuse, we are obligated by law to take reasonable precautions to ensure safety of yourself or those at risk.  Courts may also subpoena treatment records.

All appointments MUST BE CANCELLED with 48 HOURS notice. If, however, an appointment is scheduled on a Monday, notice must be provided by NOON on the Friday before the session. Otherwise I will be charged $185 for the missed session, which CANNOT be submitted to my insurance for reimbursement.
